Our RNs visit clients in their homes and provide initial assessment and reassessments of the physical, psychosocial, and environmental needs of clients and their families. Our nurses develop appropriate and individual care plans, collaborating with our clients’ other healthcare professionals as needed, teach/counsel clients regarding self-care and evaluate clients’ responses to care provided.

Our nurses also act as the clinical care coordinator for assigned individuals/ families, initiating referrals as necessary, supervising and directing caregivers regarding the care of our clients.
